National PACE Association

National PACE Association is an association in the United States which promotes home care for senior citizens who need chronic care as an alternative to institutional care.
PACE stands for Programs of All-inclusive Care for the Elderly.
The National PACE Association is a membership driven advocacy and quality improvement organization that represents over 116 organizations, that care for over 35,000 individuals in 32 states. These Programs of All-inclusive Care of the Elderly (PACE) provide a comprehensive array of services to frail adults over age 55 who would be, could be or should be in a nursing home, if not for the services and support provided by the programs. While home care is one of the provided services, this is not merely a home care program. Instead it is a managed care organization that not only provides the full array of both Medicare and Medicaid services available to frail seniors, but also pays for those approved services for enrolled participants when the interdisciplinary team of professionals (IDT) identifies the services as needed. This program provides services from necessary prescription drugs and medical equipment and supplies, to adult day care and socialization, medical and nursing care, social services, rehabilitation therapies and short term or long term nursing home placement and even hospitalizations, diagnostic testing, surgeries, medical specialty care and other needed services. In fact, while all participants in the program must have the medical and functional needs to make them nursing home eligible, a very small percentage of PACE participants actually ever live in a nursing home short or long term. The program and its professionals partner with participants and their family caregivers to provide support in the community that prevents unnecessary hospitalizations, supports independence and wellness, encourages socialization and physical activity as a means to better health and provides necessary assistance with medical care, activities of daily living and caregiver support.
